Disease Alert Contacts

There are two types of stations:

• "Station Name" — Real-time data with alerts as weather conditions occur.
• "Station Name Forecast" — Forecasts from weather two days ahead of current time. Will send an alert when there are likely going to be disease conditions or cold weather.

Please fill out a form for each station you want to receive alerts from. If you want both real-time and forecast alerts for the same station, you will need to submit two separate forms — one for the real-time station and one for the forecast station.

Diseases and Cold Temperatures (below 35°F) — Leave the disease field blank to receive alerts for all three conditions (Botrytis, Mummy Berry, and Freezing).
